Prescriptions and the Elderly

A large proportion of our elderly take multiple medications for multiple conditions. This has led to a situation known as polypharmacy, which is defined as one person taking more than five prescriptions. This is prevalent among our elderly and it needs to stop or at least be dramatically reduced. This essay focuses on the evaluation of an anti-polypharmacy program. Stakeholders are identified, barriers and strategies are discussed, business vs. strategic planning is explained, the evaluation process is identified and justified, and the evaluation model to use with the program is discussed.
